State Health requirements per grade

2025-2026

Per Dixon Public School policy, physicals, Immunizations and vision exams are due by the first day of school to avoid exclusion. Dental exams are due by May 15th.  Please turn all medical records into the school nurse.

Pre-K/ECE

1) School Physical

2) Proof of Immunization Record

·       DTaP (4)

·       Polio (3)

·       Hep B (3)

·       MMR (1)

·       Varicella (1)

Kindergarten:

1) School Physical

2) Eye Exam (by licensed optometrist)

3) Proof of Immunization record

·       DTaP (4) or more

·       Polio (4) or more

·       Hep B  series of (3)

·       MMR (2)

·       Varicella (2)

4) Dental Exam (turned in by May 15th)

First Grade:

None (unless new to district, then need to provide school physical and up to date Immunization record)

Second Grade:

1) Dental Exam (turned in by May 15th)

2) No other requirement unless new to district, then need to provide school physical and up to date Immunization record

Third grade:

None (unless new to district, then need to provide school physical and up to date Immunization record)

Fourth Grade:

None (unless new to district, then need to provide school physical and up to date Immunization record)

Fifth Grade:

None (unless new to district, then need to provide school physical and up to date Immunization record)

Sixth Grade:

1) School Physical

2) Proof of up-to-date immunizations record including

·       Meningococcal (1) on or after their 11th birthday

·       Tdap (1)

3) Dental exam turned in by May 15th

Seventh Grade:

None (unless new to district, then need to provide school physical and up to date Immunization record)

Eighth Grade:

None (unless new to district, then need to provide school physical and up to date Immunization record)

Nineth Grade:

1) School physical

2) Proof of Up-to-date Immunization record (nothing new)

3) Dental exam turned in by May 15th

Tenth Grade:

None (unless new to district, then need to provide school physical and up to date Immunization record)

Eleventh Grade:

None (unless new to district, then need to provide school physical and up to date Immunization record)

Twelfth Grade:

1) Vaccine (need proof of 2 Meningococcal vaccines)

Parent Consent forms are available online. These forms need to be filled out each year for medical concerns. Also, if you want your child to take Tylenol or Ibuprofen as needed, at school.
